Street Research, Inc. ("PSR") has a firm commitment to safeguard
your privacy online.
In
general, PSR will never share your Personal Information (as
defined below) with anyone without your knowledge and permission. We
will never share with anyone the information or opinions you
provide us regarding our client's promotional or educational
materials in any way that personally identifies you without
your knowledge and permission. Any of your information
we share with clients will be in the form of Anonymous Information
(as defined below).
By
using PSR's website ("Site") and participating in its operation
("Services"), you acknowledge that you have read our Privacy
Policy below, and you consent to our collection, use and disclosure
of your information as described below. Further,
this Privacy Policy is incorporated into and subject to the
term of our Terms of
Service Agreement.
The
purpose of our Privacy Policy is to inform you about:

information we gather from you when you become a member of
the PSR physician panel by registering to use our services
and products,
- How
we may use and disclose such information (our policies),

your options and rights are with respect to such information.
Changes
in this Privacy Policy
We reserve the right to reasonably change, modify, add or remove portions of
this Privacy Policy at any time. We will notify you if we alter this
Privacy Policy to change the way we collect or use Personal Information. Please
be sure to check this page periodically for such changes. PSR will never
use any Personal Information you submit under this Privacy Policy in a new
way without notifying you of such change, and giving you an opportunity to
opt-out, or otherwise prevent that new use of your Personal Information. This

1. What types of information does PSR collect
from you?
By becoming an member of the PSR physician panel, you will fill out a Registration
Form, which becomes your Member Profile and contains basic information aboutyou
and your practice. We collect both Anonymous and Personal Information(as
defined below) from you as an Advisor, but only after you consent to suchuse
of your information or provide it to us voluntarily.
We
collect Anonymous and Personal Information from you as a panel
member: (1) to allow you to have access to and participate in
the activities of the Site; (2) for editorial and feedback purposes,
(3) to analyze our users' behavior, and (4) to improve our Site
operations.
Other
than the uses described in this Privacy Policy, and unless we
notify you in advance (through placement of a notice on the Site)
and obtain your permission, we do not share, rent, or sell any
of your information to any outside parties, except to the extent
necessary to comply with applicable laws, police investigations,
or in legal proceedings where such information is relevant.
2. What is Personal Information and how will
PSR use it?
"
Personal Information" includes (1) personally identifiable
information,such as your name, phone number, mailing address,
email address, etc., (2) demographicinformation, such
as your age, gender, etc., and (3) your responses to promotionaland
educational surveys and questions.
As
a PSR panel member, you will be given the opportunity to provide
Personal Information so that we can determine which Site activities
are most appropriate for your participation.
For
example, certain PSR clients may wish to interact with certain
panel members, such as physicians with a particular specialty
(e.g. cardiology) or those with certain demographic criteria
(e.g. number of years in practice).
In order to become a PSR panel member and access the Site, you will be asked
to complete our Registration Form, which will serve as your Member Profile. You
may also, at your option, indicate to us whether we may contact you regarding
promotional opportunities or materials or provide you with certain value added
services, such as receiving product samples, scheduling a representative visit,
participating in continuing medical education courses, receiving invitations
to market research and symposia, being invited to speak for a PSR client company,
or participating in surveys or other services or areas of interest yet to be
determined.
Any
information that personally identifies you is disclosed by you
on a voluntary basis and only collected by PSR with your knowledge
and permission. You may, at any time, permanently or temporarily
opt-out of receiving such emails and future promotional mailings
by changing your options in your Member Preferences. See
your Opt-Out Choice below. You may also change any other information
in your Member Profile at any time.
PSR
will not disclose any Personal Information with any client
or any other third party without your knowledge or permission. If
you request or agree to receiving additional products or services
(as described above), we will ask for your permission to disclose
your Personal Information, such as your name and address, to
facilitate such a mailing. In no cases will any of
your individual answers or responses ever be made available
to any of our clients or any other third party without your
knowledge or permission. Your Personal Information and
specific answers will never result in any solicitation or offer
by PSR or any third party, including our clients without your
knowledge or permission.
In
addition, we collect Personal Information from you if you contact
us with questions or requests for help (i.e. technical support),
or to help diagnose problems with our server and to operate the
Site. We retain such Personal Information as a record of
such correspondence, but keep it confidential and use it strictly
to respond to such requests or technical necessities.
3. What is Anonymous Information and how will
PSR use it?
"
Anonymous Information" means information that cannot be traced
back specificallyto you as an individual. The Anonymous
Information we use comes from collectingdata from you as a
member of our panel (i.e. Site users) on an aggregated basis(i.e.
categorized into groups of information) and converting Personal
Informationinto Anonymous Information, which is also compiled
on an aggregated basis. Weuse such Anonymous Information
to study the behavior and usage patterns of ourpanel members,
and report data from such Anonymous Information to our clients.
While
you are logged onto the Site, we collect Anonymous Information
about what website you came from, what parts of the Site you
visit, the time of your visit, the type of web browser you use
(the software you use to connect to the Internet and visit websites),
the type of computer system you use, etc. This Anonymous
Information comes from your web browser which sends the information
to our web server in the form of URLs (website address known
as the Uniform Resource Locator), referrer Ids (information from
your web browser showing us what URL you came from), IP addresses
(a number, known as an Internet Protocol address, that identifies
your computer so that data can be sent to your computer), computer
system information and logs of web browsing activities (known
as "web logs").
In
addition, PSR may use "cookie" technology to capture your activity
on the Site. A "cookie" is a small file that is sent from our
web server and stored on your computer hard drive while you are
visiting our Site. Cookies store and retrieve information
on your computer about your web browsing activities, such as
what website you came from and what web pages you go to on our
Site.
Using
Anonymous Information helps us improve our product and service
offerings, administer our Site, and diagnose problems with our
server, services and software. Such information enables
us to make better decisions about how we operate or improve our
Site from both business and technical perspectives.
4. With whom and how is your information being
shared?
PSR does not disclose any information that could specifically identify you
as an individual unless we have your express permission prior to such a disclosure,or
as unless otherwise specified in this Privacy Policy. If you do notwant
your Personal Information to be shared with or used by an outside party,you
can choose not to allow this use by telling us when you receive and respondto
our notice of such a disclosure.
We
only report data to our clients using Anonymous Information.Your
Personal Information and your individual opinions and answers
will never be shared with our clients.
5.
How is your information used for online surveys or promotions?
PSR conducts surveys and promotions through its Service to show selected physicians
promotional and/or educational materials, and to better understand our panel
members' wants and needs regarding the services and products which may be offered
through the Site. For example, we may show you client promotional materials
and ask your opinion about the advertising, or your practice patterns, treatment
goals and objectives, etc. We will use your answers to customize and
personalize your individual experience and tailor content for you in subsequent
interactions between you and the Site.
PSR will never share your personal information or your specific opinions and
answers with our client. We will however convert certain opinions and
answers into Anonymous Information to make available to our clients.
6.
Data Security: How does PSR protect and keep your information
secure?
Our Site has industry standard security measures in place to help protect the
loss, misuse, and alteration of information under our reasonable control, and
safeguard the confidentiality of information personally identifying you. As
a member of our physician panel, your Member Profile and Member Preferences
are password-protected so that only you may access or change this Personal
Information. We therefore highly recommend that you do not disclose your
password to anyone. Also, remember that PSR will never ask you to
divulge your password in written or electronic communications.
Please
be aware that data transmission over the Internet is not always
100% secure, and we cannot warrant that information you transmit
using the PSR service is 100% secure. We will, however,
use all reasonable efforts to maintain the security of your information
while it is located on our systems. Such efforts include
using technology that encrypts any of your Personal Information
while being transmitted from your computer to our system.
7.
What types of use and disclosure of information is PSR responsible
for?
PSR does not pre-screen any communications, emails or any content distributed
through our Site by our panel members. Therefore, by using the Site and
any of the Advisor services provided, you acknowledge that PSR is unlikely
to be aware of violations of law or other misconduct committed by panel members
or any other parties which occur through our Site or third party websites and
resources linked to or framed within our Site. You also acknowledge that
you are completely responsible for any information you send via e-mail, or
any other form of communications to or through our Site.
PSR
may also reference, recommend or provide links to third party
websites, and make all reasonable efforts to link to sites sharing
our high standards and respect for privacy. However, whether
we recommend such resources or not, we do not control and therefore
are not responsible for such third party websites and their content,
information or privacy policies. Consequently, please
take the time to review the privacy policies and any other information
practices posted on such third party websites.
Please
keep in mind that PSR may be required by law, court order, governmental
agency or law enforcement authority to disclose your Personal
Information, including but not limited to email information temporarily
residing on our servers. In addition, PSR may disclose a panel
member's Personal Information to the extent necessary for emergency
situations, public health activities, health oversight activities,
or to comply with applicable laws, police investigations, law
enforcement authorities investigating suspected illegal activity,
legal proceedings (where such information is relevant) or to
PSR's legal advisors in connection with a suspected violation
of PSR's Privacy Policy, Terms of Service Agreement, User Rules,
or any other rules and regulations.
8.
How do you change, delete or prevent disclosure of Personal
Information you disclosed to PSR?
You will have the opportunity to review and change your information in our
Registration Form prior to completing the registration process. Also,
you can, at any time, access all and change any of your Personal Information
that we collect online and maintain by visiting Your
Account to make changes and additions to your Member Profile or Member
Preferences.
To protect your privacy and security regarding your Personal Information, we
will take reasonable steps to verify your identity before granting you access
to and making corrections to such information (i.e. a password-protected system).
If
you do not want to receive PSR's emails, future promotional mailings
or notices, you may opt-out of receiving such communications
from us by selecting the appropriate options during the registration
process or by editing your Member Profile or Member Preferences
located at Your Account. You
may also terminate your relationship with PSR at any time by
sending an email to Questions.
9.
Your Acceptance of The Terms of This Privacy Policy.
This Privacy Policy is incorporated into the terms of PSR's Terms
of Service Agreement. Your use of our Site is subject to such terms and
conditions, which constitute a legal agreement between you and PSR. Each
time you use the Park Street Research Site, you signify that you have read,
understood and agree to be bound by the Privacy Policy, the Terms of Service
Agreement and User Rules.
If
you do not agree to the terms of this Privacy Policy, please
do not provide us with any information and do not access the
Park Street Research Site. Your continued use of the
Site after we notify you of any changes to this Privacy Policy
will signify your acceptance of and agreement to such changes. Please
review our Privacy Policy, the Terms of Service Agreement and
User Rules, and if you do not agree to the any of the provisions,
please do not use our Site, as you do not have our permission
